Terms of Service

These Terms of Service ("Terms") are a legal agreement between you (either an individual or the company you represent, "you" or "Customer") and Universal Forge 2025, Inc., doing business as All Purpose Forms ("All Purpose Forms," "we," "us," or "our"), governing your access to and use of the All Purpose Forms platform, including the website, form-builder application, embeddable forms, and related services (together, the "Service").
By creating an account, starting a trial, or otherwise accessing or using the Service, you agree to be bound by these Terms. If you are accessing the Service on behalf of a company or other entity, you represent that you have the authority to bind that entity, and "you" refers to that entity. If you do not agree to these Terms, do not use the Service.
1. The Service
All Purpose Forms lets you design, publish, and embed online forms; collect and manage submissions, including electronic signatures; and connect submissions to third-party destinations such as webhooks, Google Docs, and Google Sheets. We may add, change, or remove features from the Service at any time, and may offer new features subject to additional terms.
2. Accounts and Registration
You must provide accurate, complete information when creating an account, and keep it up to date. You are responsible for maintaining the confidentiality of your account credentials and for all activity that occurs under your account. Notify us promptly at [email protected] if you suspect any unauthorized use of your account.
You may invite other users to your account with different roles (for example, owner, admin, editor, or viewer). You are responsible for the actions of everyone you invite, and for assigning roles appropriately.
3. Subscriptions, Trials, and Billing
Certain features of the Service require a paid subscription. Where offered, a free trial period lets you evaluate the Service before any payment is due; if you do not subscribe to a paid plan before your trial ends, your account and access to its data may be suspended.
Paid subscriptions are billed in advance on a recurring basis (for example, monthly) until cancelled. Fees are non-refundable except where required by law or as we otherwise state in writing. You authorize us (or our payment processor) to charge your chosen payment method for all fees due. You're responsible for keeping your billing information current; a failed payment may result in suspension of your account until the balance is resolved.
You may cancel your subscription at any time; cancellation takes effect at the end of your current billing period, and you will not be charged for subsequent periods. We may change our prices with reasonable advance notice, effective for your next renewal.
4. Acceptable Use
You agree not to use the Service to:
- Collect, store, or transmit content that is unlawful, fraudulent, defamatory, obscene, or that infringes any third party's intellectual property, privacy, or other rights;
- Build or distribute malware, or attempt to gain unauthorized access to the Service, other accounts, or our systems;
- Interfere with or disrupt the integrity or performance of the Service, or probe, scan, or test its vulnerability without our prior written consent;
- Use the Service to send unsolicited communications in violation of applicable law; or
- Resell or provide the Service to third parties as your own competing product, except as reasonably contemplated by your own use of forms with your own customers.
We may suspend or terminate accounts that we reasonably believe violate this section, with notice where practicable.
5. Your Content and Data
"Your Content" means the forms you build, and the submissions, signatures, files, and other data collected through them. As between you and us, you retain all ownership rights in Your Content. You grant us a limited license to host, store, process, transmit, and display Your Content solely as necessary to provide, maintain, and support the Service (including delivering submissions to integrations you configure).
You are solely responsible for Your Content and for the forms you create, including: obtaining any consents required from the people who submit data to your forms; the accuracy and legality of the questions you ask and how you use the answers; and complying with all laws applicable to the data you collect (including, where applicable, data protection and privacy laws covering your own customers or site visitors). As between the parties, you are the controller of any personal data you collect through your forms, and we act as a processor/service provider acting on your instructions with respect to that data.
Electronic signatures. The Service lets you collect a drawn or typed-name signature together with a timestamp, IP address, and the consent text shown at signing. We do not represent or warrant that any particular signature, consent flow, or record produced by the Service will satisfy the requirements of the U.S. ESIGN Act, UETA, eIDAS, or any other electronic-signature or electronic-records law applicable to your use case. You are responsible for determining whether the Service's e-signature features are legally sufficient for your particular transaction, and for seeking your own legal advice where it matters.
We may remove Your Content that we reasonably believe violates these Terms or applicable law.
6. Third-Party Integrations
The Service can send submission data to third-party destinations you configure, such as webhooks you control, Google Docs, or Google Sheets. Your use of any third-party service connected through the Service is governed by that third party's own terms and privacy policy, and we are not responsible for how a third-party destination handles data once it's delivered there. You are responsible for configuring these integrations correctly and for any consequences of sending data to a destination you specify.
7. Intellectual Property
The Service, including its software, design, and all associated intellectual property (but excluding Your Content), is owned by Universal Forge 2025, Inc. and its licensors. These Terms do not grant you any right to our trademarks, logos, or brand features. Except for the rights expressly granted to you here, all rights are reserved.
8. Disclaimers
THE SERVICE IS PROVIDED "AS IS" AND "AS AVAILABLE," WITHOUT WARRANTIES OF ANY KIND, WHETHER EXPRESS, IMPLIED, OR STATUTORY, INCLUDING WITHOUT LIMITATION WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, TITLE, AND NON-INFRINGEMENT. WE DO NOT WARRANT THAT THE SERVICE WILL BE UNINTERRUPTED, ERROR-FREE, OR SECURE, OR THAT ANY PARTICULAR FORM, SIGNATURE, OR INTEGRATION WILL BE LEGALLY SUFFICIENT FOR YOUR PURPOSES.
9. Limitation of Liability
TO THE MAXIMUM EXTENT PERMITTED BY LAW, IN NO EVENT WILL UNIVERSAL FORGE 2025, INC. BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR ANY LOSS OF PROFITS, REVENUE, DATA, OR GOODWILL, ARISING OUT OF OR RELATED TO YOUR USE OF THE SERVICE, EVEN IF WE HAVE BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES. OUR TOTAL LIABILITY ARISING OUT OF OR RELATED TO THESE TERMS OR THE SERVICE WILL NOT EXCEED THE AMOUNT YOU PAID US IN THE TWELVE (12) MONTHS BEFORE THE EVENT GIVING RISE TO THE CLAIM.
10. Indemnification
You agree to indemnify and hold Universal Forge 2025, Inc. harmless from any claims, damages, liabilities, and expenses (including reasonable attorneys' fees) arising out of: Your Content; your use of the Service in violation of these Terms or applicable law; or your violation of any third party's rights.
11. Term and Termination
These Terms remain in effect while you use the Service. We may suspend or terminate your access if you materially breach these Terms and do not cure the breach within a reasonable time after notice, or immediately for serious violations (such as illegal use or non-payment). You may stop using the Service and cancel your subscription at any time. Upon termination, your right to access the Service ends; we may delete Your Content after a reasonable retention period, and are not obligated to retain or provide it to you afterward except as required by law.
12. Changes to These Terms
We may update these Terms from time to time. If we make material changes, we will provide reasonable notice (for example, by email or by posting a notice within the Service) before the changes take effect. Continued use of the Service after changes become effective constitutes acceptance of the revised Terms.
13. Governing Law

14. General
These Terms, together with any order form or plan description you agree to, are the entire agreement between you and us regarding the Service, and supersede any prior agreements on this subject. If any provision is found unenforceable, the remaining provisions remain in full effect. Our failure to enforce a provision is not a waiver of it. You may not assign these Terms without our consent; we may assign them in connection with a merger, acquisition, or sale of assets.
